Use the questions to prompt your thoughts on what you would like to share. Of course, not all questions will fit your story, but most will, so please feel free to add to the information and make it your own.

We highly recommend setting aside 15-20 minutes so you can fully complete this form in one sitting. Sometimes the form doesn't save properly and we certainly would not want to lose any of your work. Another tip is to have your photo files ready to upload before you start filling out the form. This will save you time, as you can simply upload them right away while you complete the form.
